Ascendant Voidcores are a new system, released in Patch 12.0.5 of the Midnight WoW Expansion, that can be used to increase the item level of fully upgraded Hero or Myth track weapons and trinkets, as well as max rank crafted weapons and trinkets. Fully upgraded Myth gear will be increased to item level 298 and fully upgraded crafted gear increased to 295.
If you are seeing players with 295 or 298 item level gear and are looking for how to upgrade your own Weapons and Trinkets to item level 295 and 298, then this guide will help. It will require you to unlock access to this new system and to then participate in more end-game content to collect 5x Ascendant Voidshards to combine into an Ascendant Voidcore, which can then be used to increase your Weapons and Trinket item levels.
Unlocking The Voidforge
The first step to gaining the new Ascendant and Nebulous Voidcores is to unlock the Voidforge. This comes in the form of a repeatable quest that can be picked up from Decimus who can be found in the Voidstorm.

These quests require you to do a number of end-game activities such as Mythic+, Bountiful Delves or Raiding and to keep returning to Decimus to complete the quest before picking up the next quest and doing the same again until the questline is completed and the Voidforge is available.

Unlocking Ascendant Voidcores
Once you have unlocked the Voidforge you will gain a new quest from Decimus called New Tools, New Heights. This quest asks us to assist him in crafting the Ascendant Nilhammer.
Decimus sends you on another 4-part questline. Similar to the Building The Voidforge weekly quest, we will be asked to complete endgame content in order to gain Hungering Oblivium.
Note: Originally this questline was weekly, across 4 weeks, but now you can do these quests back-to-back. You will not be able to obtain any Ascendant Voidcores until these quests are completed.
Unlocking the Ascendant Nilhammer and completing each quest is warband wide, meaning all of your characters can then start to loot Ascendant Voidshards.
Collecting Ascendant Voidshards to Create Ascendant Voidcores
After helping Decimus craft the Ascendant Nilhammer players and their warbands will be able to start collecting Ascendant Voidshards, 5 Ascendant Voidshards combine into an Ascendant Voidcore. Ascendant Voidshards drop from endgame content such as; raids, Mythic+ dungeons, Bountiful Delves, and Nightmare difficulty Prey Hunts.

One of your characters, the first one that accepts the quest, will be rewarded with 1 Ascendant Voidshard to get your started, so its advised that you do this on your main character. However, with how farmable they are and there being no cap on how many you can collect, it's not something to be too concerned about.
Using Ascendant Voidcores
It is recommended that you use your first Ascendant Voidcore on your weapons, as this will very likely give you the largest power increase that you could gain, then your trinkets.
Note: Ascendant Voidcores can only be used on fully upgraded weapons and trinkets. This means you will need to have your intended item that you want to upgrade at the 6/6 Myth, 6/6 Hero track or the maximum crafted level.
There is no cap to the amount of Ascendant Voidshards or Ascendant Voidcores you can collect.
It is recommended that you use your Ascendant Voidcores on Myth gear first. But because of how farmable the Voidshards are, you can use them on Hero Track gear without much risk at all, since you will likely have looted enough Voidshards to make and use an Ascendant Voidcore on a Mythic weapon or trinket that you loot later. And if not, with four Tier 11 Delves being available daily, and Heroic raids being much easier by this point of the season, it won't take you long to get enough.
Once you have used an Ascendant Voidcore on an item that item will become Ascendant Voidforged preventing it from any further upgrades.
