Horrific Visions are being revisited in Patch 11.1.5 of The War Within (TWW), with a few changes from the original versions released in Battle for Azeroth. Here we have broken down the best way to tackle the Horrific Vision of Orgrimmar.
We do not cover the Hourglass in this guide, however, if you are looking for how to best progress the Hourglass you can check out our Hourglass of Horrific Visions Overview Guide.
We have broken down each area of the Orgrimmar Horrific Vision (Revisited), and have highlighted anything you may want to take advantage of in each area. The order that we recommend you progress through Orgrimmar is the order of the sections in this guide.
Valley of Strength
When you first enter the Horrific Vision of Orgrimmar you will start in the Valley of Strength. The base sanity drain in the area is 6 sanity per second. There are a few different things you can do in the area.
Our first recommendation is to head to the hut on the left where there is a dead Voidbound Ravager. Next to the corpse is the potion that reduces sanity by 100, do not pick it up. All potions that have the same colour as this potion will also reduce sanity by 100, so avoid this colour of potion when clearing the rest of the vision.

Next you can grab 1 of 2 buffs, either by entering the inn or the forge, only one of these will be active at a time. If Gamon is active in the inn, when defeated he will grant 10% increased health. If Naros is active in the forge he will grant 10% increased damage when defeated.
Odd Crystals
One location of note inside the Valley of Strength is Zarhaal, he will trade odd crystals you can find around the Horrific Vision for Displaced Corrupted Mementos. The more odd crystals you collect the more mementos you get.
The Table below shows how many mementos you get for the crystals you hand in, the more you collect in a single go, the more mementos you get; so it is better to collect as many as you can before turning them in, rather than turning them in with only a few at a time, whilst also not failing to turn them in before the Horrific Vision ends (by defeating Thrall or losing all of your sanity).

Valley of Wisdom
Valley of Wisdom is the first lost area we recommend you visit. In this zone you will lose 12 sanity per second without any unlocks on the Hourglass of Horrific Visions.
There are 2 objectives in this area, the first is to fill a progress bar to 100% and the second is to defeat Vez'okk the Lightless.
Filling the progress bar is as simple as defeating enemies in the area, you need to kill most enemies in the area including both elite mobs. We recommend taking the Coagulated Horror first, it has the following abilities:
- Sanguine Residue - Launches blood at all players leaving pools on the ground that drain 30 sanity per second and deals shadow damage. Coagulated Horror is healed for 100% of the damage dealt.
- Visceral Fluid - Frontal cone that deals heavy damage and drains 60 Sanity if hit.
After defeating Coagulated Horror you can move up to the Voidcrazed Hulk. The Voidcrazed Hulk has the following abilities:
- Explosive Leap - The Voidcraze Hulk picks up a player and throws them dealing 40 Sanity damage immediately, he then leaps at them dealing heavy damage and 60 sanity damage.
- Howling Pain - Deals Minor group wide damage and interrupts anyone currently casting.
Once you have done the first objective, the boss Vez'okk the Lightless will spawn near the entrance, Vez’okk has significantly more health than the two mini bosses so we would recommend using any damage cooldowns to kill them quickly.
Vez’okk has a few abilities we have outlined below:
- Defiled Ground - A frontal that deals heavy damage, 60 sanity damage and stuns you for 3 seconds.
- Unleash Corruption - Spawns a circle around themselves, deals damage and drains 60 sanity if any players are caught in the circle.
- Ring of Chaos - Vez’okk summons a ring of circles around themselves, then launches them outwards, deals shadow damage, drains 60 sanity and stuns the player for 3 seconds.
Valley of Spirits
Valley of Spirits revolves around freeing shamans from corrupted totems, this area has 8 Sanity drain per second, and there are two bosses you need to defeat. To enter the main area you will need to defeat Decimator Shiq'voth, he has a few abilities:
- Surging Fist - Charges towards a player deals physical damage and knocks players away. This is always followed up with a Decimator.
- Decimator - Launches a projectile at a player, dealing a small amount of damage, it then returns to Shq’voth, dealing additional damage and draining 120 sanity.
Once you enter the Valley you may find Bwemba, defeating Bwemba Grants 10% Haste and Movement speed. Bwemba has 2 abilities:
- Hex - Instant cast turns a player into a frog for 4 seconds.
- Twisted Lightning - 3 second cast chain lightning.
The next step is to free the shamans from the corrupted totems, to do this you simply need to defeat the enemies around the totem and then click on the totem. Once all four shamans are free, you can take on the Oblivion Elemental. The Oblivion Elemental has a couple of abilities:
- Waves - While in combat, the oblivion elemental spawns waves that move through the area, these do minor damage but also drain sanity when hit.
- Hopelessness - at 80% and 40% health the Oblivion Elemental will cast Hopelessness, this causes an orb to spawn out of the player that you need to collect. You will be pacified until you collect your orb.
The Drag
The Drag revolves around checking for survivors in the various shops, this area drains 8 sanity per second. There are 2 bosses you need to defeat in the area. When you first enter the area you will need to defeat Annihilator Lak'hal, they have 2 abilities:
- Orb of Annihilation - Spawns an Orb in front of him that explodes being caught in the effect, deals heavy shadow damage and drains 60 insanity.
- Dark Force - Lak’hal targets a player with a beam and locks on to that location being hit by the beam deals damage and knocks players back.
Once you have defeated Lak’hal you need to speak to Garona. Once you have spoken to her you can make your way through the drag checking each building. The transmog shop on the right may have a clickable Ethereal Portal which will summon Warpweave Dushar. Defeating Warpweaver Dushar will grant you a 10% increased critical strike chance buff.
Once you have checked all of the buildings, you will be able to face Inquisitor Gnshal. He is the boss of the area and has two abilities:
- Void Torrent - Gnshal targets a player and launches several void missiles at them over a few seconds. Being hit causes players to be stunned and lose 30 sanity every second for 4 seconds. This is much easier to dodge if you keep moving.
- Cries of the Void - At 65% and 30%, Gnshal uses Cries of the Void, this shields him for 25% of his maximum health. If the shield remains after 6 seconds players take damage and sanity drain based on the remaining value of the shield.
Valley of Honor
The Valley of Honor is probably the most difficult area in the Horrific Vision of Orgrimmar. There is only a single objective in this area, to defeat Rexxar. However, as you enter the area, Misha will block your path and you will need to handle her. Misha has the following abilities:
- Desperate Retching - Afflicts players with a disease that deals Nature damage and leaves behind a pool that deals nature damage and drains 30 sanity per second.
- Maddening Roar - Deals massive damage and 120 sanity drain in a small circle around Misha.
- Summon Aqir Scarab - On death Misha summons several Aqir Scarab.
As you move through the Valley of Honor you will want to avoid as many mobs as possible, and standing on corpses will spawn Aqir Scarabs. In the center, you will find an Aqir Mindhunter, this can be avoided with good movement. If you do accidentally pull it, it has the following abilities:
- Toxic Breath - Deals Nature damage and applies a stack of Aqiri Mind Toxin if you reach 5 stacks, you will get stunned for 3 seconds.
- Toxic Volley - Launches several bolts at nearby players, deals heavy damage and drains 30 sanity to any players caught inside the circles.
Once at the top of the hill you can fight Rexxar, this is one of the hardest encounters in Horrific Visions, so try to make sure you have cooldowns available. Rexxar has the following abilities:
- Mind Traps - At the start of the encounter Rexxar summons several traps, standing in a trap will cause you to take shadow damage, be stunned for 4 seconds and will drain 120 sanity.
- Stampeding Corruption - At 40% Rexxar summons a stream of minions the travel across the room, being hit by any of them will cause shadow damage and drains 60 sanity.
- Summon Void Boar - As Rexxars health drops, he summons boars. The boars Cast Void Quills, which deals shadow damage and drains 30 sanity. Try to deal with the boars quickly when they spawn, you want to avoid getting overwhelmed by too many of them being active.
Thrall
Thrall is the final boss of the Horrific Vision of Orgrimmar. To access Grommash Hold you need to defeat the 2 Voidbound Horror Guards that stand in front of the entrance. Depending on how many other areas you cleared, Thrall will gain 10% health and additional abilities for each. Below, we have a list of the abilities Thrall has:
- Surging Darkness - Thrall leaps to the center of the room and causes rings to spawn; the first is in the center, then they spawn towards the edge or start at the edge and head inwards. This deals huge damage and drains 60 sanity to players hit.
- Seismic Slam - Slams a small area in front of him, stuns players hit for 2 seconds and drains 30 sanity.
- Cries of the Void - Only usable if you defeated Inquisitor Gnshal, at 55% health Thrall shields himself for 25% of his max health. After 6 seconds the shield will explode dealing damage and draining sanity based on the value of the shield remaining.
- Void Boars - Only used if Rexxar is defeated, Thrall has 2 Void Boars, these are identical to Rexxars. They have low health, so we recommend defeating them as quickly as possible. Void Quills is interruptible and should be interrupted as much as possible.
- Defiled Ground - Only used if Vez’okk the Lightless was defeated, replaces Seismic Slam. It behaves in the same way, but will leave behind a void zone that deals damage and drains sanity if you stand in it.
- Hopelessness - Only usable if you defeated Oblivion Elemental. At 80% and 40%, Thrall will cast Hopelessness. Silences all players for 10 seconds or until they collect the golden orb that spawns in the area.
