With the reintroduction of Horrific Visions in Patch 11.1.5, Blizzard have introduced the Hourglass of Horrific Visions; these are upgrades for your character to more easily progress through the harder Horrific Visions (with up to 8 masks active).
Note: Horrific Visions are only available after the weekly reset of May 20, 2025.
If you are looking for the best Horrific Visions Build, in this guide we have broken down the order we recommend you upgrade the Hourglass to maximise how quickly you can get through Horrific Visions with more masks active. This will mean you can collect Displaced Corrupted Mementos faster, upgrade the Hourglass sooner and therefore collect the rewards available from Horrific Visions earlier.
Hourglass of Horrific Visions Location
The Hourglass of Horrific Visions is located in Dornogal near the entrance to the Ringing Deeps. To upgrade you will need to interact with the Hourglass.

How to Upgrade the Hourglass of Horrific Visions
Upgrading the Hourglass requires Echo of N’Zoth, these are obtained from Augermu who is standing next to the Hourglass in Dornogal. The first time you purchase an Echo of N’Zoth it will cost 100 Displaced Corrupted Mementos, then every time you purchase an Echo of N’Zoth the price increases by 200 Displaced Corrupted Mementos.
Displaced Corrupted Mementos are obtained by defeating enemies and bosses within a Horrific Vision.
Optimal Upgrade Path for the Hourglass of Horrific Visions
Our recommended upgrade Path for the Horrific Visions Hourglass is based on staying in Horrific Visions as long as possible, this will increase the amount of Displaced Corrupted Mementos you can collect, the upgrade order is as follows:
- Orb Operation Manual
- Steeled Mind
- Singular Sanitation Expertise (Solo) / Expansive Mind (Group)
- Steeled Mind 2
- Steeled Mind 3
- Singular Sanitation Expertise 2 (Solo) / Expansive Mind 2 (Group)
- Singular Sanitation Expertise 3 (Solo) / Expansive Mind 3 (Group)
- Emergency Cranial Defibrillation
- Steeled Mind 4
- Synchronized Mind Stabilize
Upgrade Order Discussion
Upgrading the Hourglass of Horrific Visions becomes more and more expensive, so selecting the right upgrade path is important. When you first open the Hourglass you will have to select the Orb Operation Manual node.
You will then have 3 options, either Singular Sanitation Expertise, Steeled Mind or Expansive Mind. We recommend your first option to be Steeled Mind as reducing the Sanity Drain will let you stay in horrific visions longer.
Your 2nd purchase has to be either Expansive Mind or Singular Sanitation Expertise. If you are playing solo we recommend taking the Singular Sanitation Expertise node, this increases your damage while you are alone, making it much easier to kill mobs and bosses inside the horrific visions, but if you are currently playing with a friend, then Expansive Mind can be a good choice as this increases the amount of sanity you currently have.
For your next 2 purchases we recommend going for Steeled Mind 2 and 3. By reducing the sanity drain it will increase the amount of time you can spend in the Horrific Vision. Then, if you are playing solo, fill out the left hand side of the tree as this will increase your damage while on your own, meaning you will be able to kill mobs and bosses much faster, or the right side if you are playing in a group as this will increase the amount of sanity you have, meaning you can stay in Horrific Visions longer.
For the Final 3 points, we recommend purchasing Emergency Cranial Defibrillation, this gives you a free auto res if you run out of sanity or health. Followed by Steeled Mind 4, this like the previous Steeled Mind selections reduces your sanity drain, meaning you can spend more time in the Horrific Visions. Finally, Synchronized Mind Stabilizer, as this makes the first res in your group free; while useful, we feel the other options are much better and that is why this is the last node we unlock in our recommended best Horrific Vision build.
We think this unlock order will increase the time you can spend inside Horrific Visions and increase the amount of Displaced Corrupted Mementos you can obtain. Once you are fully upgraded you can spend your Displaced Corrupted Mementos for Mounts and Pets.
